He would have to be certified insane. I suspect he thinks everything's just fine in his demented little world, so he hasn't seen the need for professional help.

And if he hasn't said this to anyone else before, why would they suspect a thing?

True, if he hasn't displayed any signs of madness before now then no one would suspect anything. However, I'm really hoping that's changed.
